FUNCTION:

Develop, implement, monitor, and assist in evaluating HIV/STI/Hepatitis prevention, screening, navigation and support services to LGBTQ+ communities of color, with a focus on MSM and transgender men and women.

 

ESSENTIAL DUTIES:

  1. Engage LGBTQ+ communities of color through in-person/virtual outreach activities. 

  2. Provide HIV/STI/Hepatitis testing, navigation and referrals to care.

  3. Provide support services to clients living with HIV and/or Hepatitis.

  4. Provide HIV/STI/Hepatitis individual and group services, both in-person and via virtual platforms.

  5. Sustain relationships within LGBTQ+ community, including leaders, influencers, and agencies serving LGBTQ+ people of color.

​RESPONSIBILITIES:

  1. Coordinate with all MAC programs to develop and disseminate prevention/education materials and messages, recruit and assess clients, provide referrals and navigation services, implement programs as appropriate.

  2. Conduct HIV/STI/Hepatitis counseling and testing as required.

  3. Conduct assessment and enrollment for peer support and case management services.

  4. Develop content for social media outreach, recruitment, and prevention education, ensuring dissemination at least weekly through various platforms.

  5. Develop and maintain appropriate collaborative relationships within the community, serving as liaison to health care providers.

  6. Implement individual and group Health Education/Risk Reduction interventions.

  7. Provide individual and community education on PrEP.

  8. Participate in agency meetings, program planning, and strategic planning processes.

  9. Ensure client confidentiality and maintain secure client files.

  10. Organize monthly data reports and preparing contract reporting.

  11. Participate in professional development, training, workshops and/or courses to increase individual capacity and industry awareness

  12. Other duties as assigned.

 

​SKILLS REQUIRED:

  • Strong communication and time management skills

  • Ability to work as part of a team.

  • Ability to work independently.

  • Ability to utilize and manage social media

  • Knowledge of HIV prevention, including biomedical HIV prevention strategies

  • Certification in HIV counseling and testing/willingness to be trained

  • Group facilitation skills

 

​QUALIFICATIONS:

  • BA in health, human service or related field or 3 years related experience. 

  • Experience in working with LGBTQ+ communities.

  • Experience in HIV/STI/HCV screening, prevention, education.
